The Admissions and Discharge co-ordinator helps to provide a seamless care pathway for patients between the community, the acute trust and the In-patient Unit; ensuring that when helping to organise admissions, patients will be able to obtain in-patient specialist palliative care when they need it most, whether it be for symptom control, psychological support or end of life care.
The protocol for admission and discharge will be closely followed and the Clinical Nurse Specialists, with their expertise, will be the crucial link between the In-patient Unit and the patients’ home. Patients will be able to be assessed by our team of specialist doctors and nurses so that their specific needs can be addressed as quickly as possible and then be cared for in a unique, comfortable and safe environment.
