Clinical Nurse Specialist – Community Nursing

Salary: £39,975-£46,312 FTE dependent on experience and skill level

We are currently seeking a caring, compassionate, committed and professional Registered Nurse to join us as a Clinical Nurse Specialist to work within our Community nursing teams. The successful candidate will provide responsive, flexible, high quality community specialist palliative and end of life care to patients and their families.

The post holder will work autonomously in the community to provide specialist assessment of patients with complex palliative care needs, develop management options, advising others involved with the patient’s care and evaluate the plan in the face of changing needs.

You will perform comprehensive holistic nursing assessments of patients with sensitive and complex palliative care needs using a variety of methods, telephone, digital and face to face assessments. You will also provide effective, appropriate and efficient responses to external and internal referrals received for all hospice services.

The role will include working with multi-disciplinary team members, contributing to education programmes for a variety of professionals and organisations. In addition to working collaboratively with clinical colleagues to contribute to the continual development of services and quality of care delivered.

Post registration experience in a variety of settings and significant specialist palliative care experience are essential for this role and experience of working as a Community Nurse and telephone assessment of patients’ needs is desirable.

Each year the hospice team supports more than 3,100 patients, family members and bereaved people in Worcestershire.

Due to the nature of role and working throughout the Worcestershire community, a full clean driving licence and access to a vehicle are essential for this role.
