The Living Well Centre hosts specialist palliative joint supportive clinics for people affected by Parkinson’s, respiratory conditions, heart failure and renal failure.
The specialist clinics offer access to: Specialist Consultant, Consultant in Palliative Medicine, nurse specialists, physiotherapist, social worker and members of the spiritual care team. The team advises and supports patients, their families and carers on choices of treatment to control symptoms, on psychological issues and on making plans about their future care.
We work closely with GPs, community nurses, social services and hospital consultants. The clinics offer the opportunity for patients and their carers to meet others in a similar situation to share their experiences and ways of coping.
Referral to the specialist clinics can be made by your Consultant, GP or Community Nursing Team.
During this challenging time, we are doing our utmost to continue caring for the patients and loved ones who need us most.
The way we work has been adapted, with many of our services delivered remotely – such as over the phone, or by video call. And we are working closely with other health care professionals in Worcestershire to share knowledge and expertise at this time.
